Teams

Age as of August 31 for the following divisions:

2 – AAA (9-10), 2 – Majors (11-12), 1 – Junior Age as of August 31 (13 - 14), 1 – Senior (15 - 16)

What you will need:

If the player has participated in MFLL All Stars previously and has not switched schools since they last played and no other changes have occurred:

• Player verification form

If the player has participated in MFLL All Stars previously and has switched schools since they last played:

• Player verification form
• School enrollment form

If the player has not participated in MFLL All Stars previously:

• Original Birth Certificate (no copies allowed, the original will be returned at the end of the tournament)
  • Tournament Player Verification
  • School Enrollment Form – dated prior to October 2018 for the 2018/2019 school year with the physical address of the school attended listed

OR

• Tournament Player Verification form with name and address along with one item from each of the three groups on the form.  Use the 2019 Player Validation Requirements as your guide ensuring that:
  • the name and address matches on all documentation provided
  • you did not use the same document to satisfy multiple groups

Exceptions

2D form - if moved outside of the boundaries but maintained continuous participation, this form should be completed in addition to the Tournament Player Verification

VERIFICATION - SIGNATURES NEEDED

Print, Sign and Date the Tournament Player Verification form

No need to fill in Date Requested or League ID#

Lastly - Keep a copy of each document for your records

Managers

Upon receiving the information from your players, review the documentation provided and complete the Tournament Verification Checklist to ensure the information is applicable checking for accuracy of residency, names, dates, age of player etc.  Age is determined by the players birthday as of August 31.

Turn your entire teams information in by 6/16.
